PDP Provider Terms & Conditions

I have read and accept the Terms & Conditions for CEC Providers as amended from time to time, and agree to be bound by them. I understand that I am required to maintain PDP Provider Registration at all times in order for my program to be eligible for, and maintain, approval. I also understand that failure to comply with the Terms & Conditions for PDP Providers may result in the suspension or withdrawal of approval for your program, and/or the suspension or cancellation of your Provider Registration.
